How do I do a websurvey?

What do I have to do to get a survey up and going 20-25 questions.
Obviously apache, and a database (Postresql), but how do I graph the results and such.
Any information would be helpful, thank you all.

Maybe I did not ask with very good details:
I am running RH6.2
Currently installing/configuring
PHP4.0.6
PostgreSQL 7.1.2
and Apache 1.3.20

I want to be able to run a web survey with this config, and I can make the page using frontpage, thats NP, but how do I interface it with PHP and SQL? Has anyone done this, or seen any documentation on this? Or is it so easy, and Im missing something that I will probably see along the way? Will I even need PHP?
